- Valid ID of member
- Original Medical Certificate
- Medical Abstract
- Statement of Account from the hospital

BASIC REQUIREMENTS FOR MORTUARY CLAIMS:
- Valid ID of deceased member
- Original Death Cert. (NSO or Local Civil Registrar)
- HCSCCO documents like passbooks and Coop ID etc.
- Other Legal documents as required.
